How does the HUD 232 LEAN program work for senior housing?

Short answer

HUD Section 232 insures FHA mortgages on residential care facilities, AL, MC , board-and-care, and SNF . The LEAN processing track standardizes underwriting and compresses review to 60-90 days post-application. 232/223(f) is the refinance of an existing licensed facility and is the up-to-35-year term. New construction and substantial rehabilitation use Section 232 and can amortize up to 40 years after interest-only construction. Loans are fixed-rate and non-recourse. Mortgage Insurance Premium (MIP) is paid annually.

HUD 232 LEAN program structure and timeline

Eligible properties: HUD 232 finances licensed residential care facilities, assisted living, memory care, board-and-care, and skilled nursing. Properties must be appropriately licensed in the state. 232/223(a)(7) refinances existing FHA-insured loans. 232/241(a) is a supplemental loan. 232/223(d) is an operating-loss loan.

LEAN processing: introduced to standardize HUD 232 underwriting and compress timelines. LEAN-approved lenders submit standardized applications; HUD review targets 60-90 days post-complete-application. Pre-LEAN HUD review took 6-12 months.

Loan terms: fixed-rate and non-recourse to the borrower. Loan-to-value up to 80% for-profit / 85% non-profit on refinance/acquisition. DSCR floor 1.45x for-profit / 1.40x non-profit (post-MIP). The borrower's note rate is the market rate agreed with the FHA lender, not HUD's published debenture rate.

Mortgage Insurance Premium (MIP): borrower pays an annual MIP, initial premium plus annual premium. MIP is part of the all-in cost calculation.

Operator approval: HUD requires the operator (which may be different from the borrower) to be HUD-approved. Operator review covers experience, licensing track record, regulatory compliance, and financial capacity. New operators face additional scrutiny; experienced operators with HUD track record clear faster.

Sponsor playbook: HUD 232 is the deepest source of long-tenor, non-recourse senior housing debt in the market. A 24- to 36-month bridge cannot land in 232/223(f) at stabilization, because 232 requires completion at least three years before application. Engage HUD-approved lender 12-18 months pre-need.
