Independent living (IL) finances closer to multifamily, agency lender options (Fannie, Freddie) plus banks. Assisted living (AL) and memory care (MC) are operating-business-heavy: HUD 232 LEAN , debt funds, and specialty banks dominate. Skilled nursing (SNF) is almost exclusively HUD 232 LEAN plus specialty lenders. Pricing widens with care intensity.
Senior housing care continuum and lender pool by segment
Independent living (IL): residents live independently with hospitality services (meals, housekeeping, activities). Operating-business component is modest. Agency lenders (Fannie Mae, Freddie Mac) finance IL under their multifamily programs at multifamily-like terms. Bank and life co also active. Pricing similar to garden multifamily plus a 25-50 bp premium.
Assisted living (AL): residents need help with activities of daily living (bathing, dressing, medication). Significant operating-business component, staffing, licensing, regulatory compliance. Agency lenders (Fannie's senior housing program, HUD 232) finance AL with operator-quality underwriting. Bank and debt fund active.
Memory care (MC): specialized AL for residents with cognitive impairment. Higher staffing ratios, secure-unit construction, specialized programming. Lender pool overlaps with AL but underwriting is more conservative because of the heavier operating model.
Continuing Care Retirement Community (CCRC / Life Plan Community): combines IL, AL, MC, and sometimes SNF on one campus. Resident pays an entrance fee plus monthly service fee. Bond financing plus HUD 232 plus bank financing, complex capital stack. Underwriting is operating-business-heavy and entrance-fee-dependent.
Skilled nursing (SNF): licensed nursing facilities with full medical care. Almost exclusively HUD 232 LEAN program plus specialty SNF lenders. Medicare/Medicaid reimbursement mix is the central underwriting issue. Per CMS data, reimbursement rates vary materially by state and have not kept PACE with operating-cost growth in some jurisdictions.
