Senior housing lenders underwrite the operator's track record (years of experience, portfolio size, geographic footprint), regulatory compliance (state survey history, deficiencies), financial capacity (audited financials, working capital), and care quality (CMS five-star rating for SNF , family satisfaction surveys). Per NIC MAP data, operator-quality dispersion is wide.
What to review about a senior housing operator
Track record: lenders evaluate the operator's years in business, number of properties under management, geographic footprint, and care levels operated. New operators face significantly more conservative underwriting; established operators with 5+ years and 5+ properties get the most favorable terms.
Regulatory compliance: state survey history is reviewed for the operator's existing portfolio. Recurring deficiencies, immediate-jeopardy citations, or license suspensions are red flags. SNF operators get CMS five-star quality rating reviewed; ratings below three stars trigger conservative underwriting or loan rejection.
Financial capacity: operator audited financials are reviewed for working capital, debt service coverage on existing portfolio, and ability to fund operating losses during lease-up or repositioning. Lender often requires operator-level guaranties or financial-covenant compliance.
Care quality metrics: family satisfaction surveys, employee retention, staffing ratios, and clinical outcome metrics are reviewed for AL/MC/SNF operators. The operating-performance gap between top-quartile and bottom-quartile operators within the same care level and metro is wide, and lenders price for it.
Operator transition risk: if an existing operator is being replaced, lenders apply additional scrutiny. Transition typically requires a defined repositioning plan, replacement-operator approval, regulatory continuity, and bridge financing for transition-period cash flow gaps.
Sponsor playbook: select operator before sourcing debt. The operator's identity drives lender selection, pricing, and execution. Sponsors with multi-operator portfolios should curate a HUD-approved operator bench for HUD 232 execution.
