How do lenders treat Medicare/Medicaid payer mix on skilled nursing?

Short answer

Lenders underwrite SNF properties more conservatively when Medicaid payer mix is high because of state-budget reimbursement risk. Per CMS data, Medicaid reimbursement rates vary materially by state and have not kept PACE with operating-cost growth in some. Lenders look at state methodology, payer-mix trends, and CMS five-star rating.

Medicare/Medicaid mix and SNF financing economics

Payer mix matters because reimbursement varies sharply: Medicare (post-acute, short-stay) reimburses well above operating cost. Medicaid (long-term custodial) reimburses near or sometimes below operating cost in many states. Private pay (resident or family pay) varies by market.

State Medicaid methodology variation: per CMS data, state Medicaid reimbursement methodologies and rate adequacy vary materially. Some states (with stronger budgets and rebasing cycles) have rates closer to operating cost; others have widened gaps. Lenders maintain state-by-state Medicaid rate adequacy assessments.

Mix-tier underwriting: an SNF with 60%+ Medicare/private pay mix gets the most favorable underwriting because reimbursement covers cost and supports DSCR. An SNF with 80%+ Medicaid mix in a low-rate state gets a higher DSCR floor, less proceeds, and sometimes a shorter amortization or a decline.

Rate-adjustment risk: state Medicaid rate freezes or cuts can compress operator margins materially. Lenders model Medicaid-rate stress scenarios (e.g., 2-3% rate freeze or 5% cut) and require adequate DSCR cushion at the stress.

CMS five-star quality rating: the rating affects both operator viability (low-rated facilities lose admissions and revenue) and Medicare reimbursement. Lenders review the rating and require minimum thresholds (typically 3+ stars; 4+ stars for tightest pricing).

Sponsor playbook: in high-Medicaid markets, focus financing on operators with strong cost discipline, recent CMS five-star ratings, and strong state-relationship infrastructure. HUD 232 LEAN remains the dominant lender for SNF; commercial lenders are selective.
